Collecting feedback from clients

Do any co-ops have any tools for collecting feedback from clients in a structured way?

We’re still quite a new co-op and we realised we didn’t have a good system for this. We basically just let the client talk about it and take notes and try and interpret them.

I wondered if there is any practical system/structure for doing this, especially if it can work via email say for those times where budget is tight.
